I installed AviSynth 2.5.8 first, then ffdshow rev3265 x64.  I selected AviSynth scripts serving and application plugin when installing.
The Codecs panel of ffdshow video decoder configuration only allows 'disabled' to be selected for AviSynth.

How can I get ffdshow x64 to work with AviSynth?

Install Avisynth64.

Probably here in developer section in Doom9 -  SEt's Avisynth 2.5.8 MT compiled for *X86_64*, Latest Build 4/16/2010
